Alien Zombie Megadeath is the sequel to Alien Zombie Death. It is a fast-paced shooter set in fenced, horizontally-scrolling 2D environments. The game consists of 80 levels where a lone spaceman has to take out alien zombies. He is able to move around, jump up and down platforms, and two separate buttons are used to shoot left or right. In addition he is able to carry certain items. While carrying something around, the gun cannot be fired. Movement is also slowed down slightly while firing.
Each level holds a different type of challenge, such as surviving waves of enemies, weaving through hazards to collect crystals, defusing bombs, escorting space babies, using decoy pods to distract large amounts of enemies, and boss fights. For each challenge up to four medals can be earned and these unlock new challenges along a path on a galactic map. Next to the main stages there are alternative, harder ones. These are often survival challenges where the game goes on until all lives are gone, instead of ending when the goals have been met. There is only one gun, but various pick-ups spawn at regular intervals: 3-way shots, a piercing laser, homing missiles, an item to slow down opponents, and health. Destroyed enemies leave behind small crystals, but the player is especially encouraged to go after large crystals that spawn for a limited amount of time and act as a multiplier each time a new one is picked up. Quickly defeating opponents also leads to multiplier bonuses.
Levels come with different types of enemies that usually spawn in waves. Some just walk around, while others can fly, shoot or carry shields. Later levels introduce additional hazards such as laser beams, meteorites, impenetrable objects and swinging lasers. For each challenge the spaceman starts with three lives and health pick-ups provide an additional one. Progressing though levels unlocks additional outfits. The game can be played cooperatively with another player through a local multiplayer mode.
